Diabetes management is the treatment of symptoms, from maintaining healthy blood glucose and blood pressure to balancing cholesterol levels and diet. Treatment plans work to minimize, delay or prevent other health complications that may arise from type 1 or type 2 diabetes. Diabetes management programs also provide education, support, and the resources necessary to improve the quality of life for those living with diabetes.
CareOne develops highly personalized diabetes care plans for each patient’s specific needs, including the cultural, religious and financial aspects of dietary management that may impact food choice or consumption. From there, interdisciplinary teams work closely together to improve each patient’s functioning and prevent potential re-hospitalizations through careful glycemic management, early identification of symptoms, and prompt treatment protocols. These teams of specialty physicians include endocrinologists, diabetic nurse educators, registered dietitians, nurse program directors and medical directors.

Additionally, our care program offers various therapeutic services with treatment up to seven days per week. This includes skilled therapy assessments, functional mobility training, medication system training, and self-care and energy conservation strategies.
