CareOne at Concord, located across Rte 2 from Emerson Hospital, offers outstanding post-acute rehabilitation and medical management programs.

The Interdisciplinary Care Team, led by a team of local physicians, work collaboratively to achieve your personalized goals. CareOne at Concord also offers a Dementia Specialty Care Unit that meets the regulations for licensing of Long Term Care facilities in Massachusetts. Our Dementia Specialty Care Unit is secure and includes a dedicated Activities Program Director, a person-centered approach to care and an environment that assists residents with Alzheimer’s Disease and Related Dementias (ADRD) to function at their best ability. Our staff receives ongoing training that focuses on the care of people with ADRD.
