This information is collected when you request additional information relating to our services and is stored in a secure location and used only for the purposes for which you provided. If you chose to offer personal information to CareOne, in no case will CareOne sell, license or transmit that information to third parties. Any information you provide to CareOne is kept confidential and secure. No personal information will be released to third parties, except to further the purpose for which you provide the information, such as requesting information, or if release is required by law or is pertinent to judicial or governmental investigations or proceedings.

Statistical information concerning the usage of the site may be collected. This information allows CareOne to monitor its utilization and continuously improve the quality of the site.

This site is not directed toward children. CareOne asks that no one under the age of 13 provide any personal information.  This site has security to protect the loss, misuse and alternation of the information under our control.

This Privacy Policy is governed by the substantive laws of the State of New Jersey.  Jurisdiction of any claims arising under or out of this Privacy Policy shall lie exclusively with the state and federal courts within the State of New Jersey.  If any provision of this Privacy Policy is found to be invalid by a Court having competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions, which shall remain in full force and effect.

  • The right to an accounting of disclosures of your health information.

We reserve the right to change our privacy practices and to make the new provisions effective for all health information we maintain.  Should our privacy practices change, we will post the changes on the bulletin board in our facility, as well as on our web site.  A copy of the revised notice will be available after the effective date of the changes upon request.

We will not use or disclose your health information without your authorization, except as described in Center Policy.
