Improving each patient's quality of life is our main focus in providing long-term care and short-term rehabilitation services. Along with meaningful activities and programs, we offer social services and care planning, weekly outings, religious services, volunteer programs and around-the-clock health care services in a comfortable, home-like setting.

For long-term care residents who have medical needs, we provide 24-hour-a-day nursing care, nutritional monitoring and planning, medication management, personal care and other medical services such as on-site podiatry, dentistry and opthamology.

Physician involvement is an important component of each resident’s care plan. Residents can choose to have their community physician or choose from a list of attending physicians.

Our long-term care planning encourages family involvement. Families are invited to attend care plan meetings with the goal of being kept informed throughout their loved one’s stay.

If you or a family member are in need of nursing and/or rehabilitation services following a recent hospitalization, South Jersey Health Care Center offers medical and physical rehabilitation. We treat patients in need of rehabilitation following orthopedic surgery, stroke, oncology care, cardiac care, general surgery and many other diagnosis.

Our post-hospitalization care features daily physical, occupational and speech therapy, as well as rehabilitative nursing and physician directed rehabilitation plans, IV therapy, wound care and several other services. Patient rooms are located on a separate, distinct rehabilitation unit. The post-hospitalization program at South Jersey Health Care Center assists each patient in regaining their strength and independence.

Family teaching and training are also available if a patient chooses to return to the home setting following rehabilitation. A discharge planner will assist in coordinating the necessary home care services to ensure a smooth transition home.

If you are considering South Jersey Health Care Center for a post-hospitalization stay, we recommend that you bring the following items with you for your stay with us: loose fitting clothing for rehabilitation, pictures of family and friends, undergarments, preferred toiletries and insurance cards.
