CareOne Staff Enhances Transitional Care Skills At Holy Name Institute For Simulation Learning

Staff members from CareOne’s Bergen County skilled nursing facilities recently participated in the transitional care simulation scenarios at the Holy Name Institute for Simulation Learning.  This Teaneck-based institute is the only hospital-based one of its kind in North Jersey.  Scenarios included live actors who re-created difficult scenarios related to transitioning from a post acute center to the next level of care.  Participants were both clinical and administrative (nurses, therapists, social workers, Administrators, Directors of Nursing) and regional team members from CareOne’s operations, compliance, education, clinical reimbursement and integrated care coordination departments.

This unique, collaborative learning experience allowed clinical leaders from each CareOne center to both interact with the actor, as well as serve on a team which debriefed each scenario.  The training allowed both individuals and teams to respond to difficult situations with the confidence and expertise that can only come with deliberate practice. The detailed instruction and post-training de-brief by the simulation team helped empower participants with the skills necessary to effectively assist patients and families in transitioning to the next level of care.
