CareOne At Hanover Township And ShopRite Of Greater Morristown Develop Health And Wellness Partnership

Whippany, NJ – August 7, 2015 – ShopRite of Greater Morristown and CareOne at Hanover Township partnered to create a Community Re-Entry Program for post-acute rehabilitation patients. The program provides patients with the opportunity to practice daily living activities, such as transferring in and out of the car and grocery shopping, prior to discharge.

Through the program, CareOne at Hanover patients will visit ShopRite of Greater Morristown with their physical or occupational therapist two weeks before their discharge date, then again the week before they return home. Therapists will provide physical and occupational therapy with patients during these visits while assisting them in re-acclimating themselves with day-to-days tasks.

Patients can also take advantage of the wide variety of services offered by ShopRite of Greater Morristown, including:

  • Free consultations with the store’s on site Registered Dietitian. “I love creating personalized shopping lists and menu ideas for our guests,” said ShopRite of Greater Morristown Registered Dietitian Jessica Guarnieri. “Whether it be low sodium, heart healthy, or diabetic, there are so many products in our store that can fit someone’s lifestyle. I can show them where these products are located and how to use them.”
  • Meetings with the Pharmacist to discuss their medications and arrange for at-home medication delivery. Patients will also have medications delivered to CareOne at Hanover Township before they are discharged. “One of my favorite parts of my job is interacting with our guests. I like to take the time to explain their medications, answer any questions they may have and inform them about all the great pharmacy programs we offer,” said ShopRite of Greater Morristown Pharmacist Audrey Wallace, PharmD. “I’m excited to partner with CareOne and make a personal connection with their patients and aid in their care and treatment plans.”
  • ShopRite from Home, which allows patients to purchase all of their groceries online with the option of picking up their purchases at a separate entrance of the store or having the order delivered straight to their home.

The new initiative was launched last week. CareOne at Hanover Township patient Roy Gartrell, his therapist Jorge Castro, OTR and Director of Rehabilitation Dominic Olivo, PT, DPT visited ShopRite of Greater Morristown, where they met with the dietitian and pharmacist and practiced grocery shopping and using Shoprite from Home. Mr. Gartrell said he enjoyed the experience and can’t wait to do it again.
