Celebrating National Case Management Week
This week marks the Case Management Society Of America’s National Case Management Week, recognizing case managers and their invaluable contributions to the healthcare industry. The case manager’s role is to advocate for their patients by helping them understand their health issues and the necessary medical strategies used to address them and by assisting their patients in navigating the often complicated insurance system. Working in partnership with each patient’s medical team and skilled nursing center, case managers help their patients achieve better health outcomes.
When working with a patient, the case manager’s primary objective is to ensure that they receive the highest quality of services within the guidelines of their insurance policy. Cheryl Wescott, CareOne at Evesham’s Director Of Case Management, works closely with the skilled nursing facility’s case management team, discharge planners and community case managers. “Our goals as a team are to make sure services are provided through authorized vendors and providers to defray out of pocket cost to the patient, enable our patients to return home at their previous level of independent functioning and prevent re-hospitalization through co-coordinating home care services and community case management follow-up,” she explained.
